Tank explodes and kills dive shop employee in Floride. Findings state that she was attempting to add 98% oxygen to the 30 cubic foot cylinder... the cylinder fell and exploded killing her and injuring 3 customers. Investigators attribute the fire and explosion to the presence of an organic contaminat. Investigators said that either the cylinder or valve or both had not been properly cleaned.
